Using a cancellation review for learning
A cancellation review should identify what can be learned without assuming every outcome was preventable.
A practical approach
Separate the customer’s stated reason from the team’s interpretation. Review whether expectations, scope and follow-up were recorded accurately. Identify the parts of the process the team can improve and the factors it could not control. Preserve the original outcome instead of rewriting it to fit a preferred explanation.
